Patient's Pain Communication Tool

One of the problems pain patients face is to accurately convey to their healthcare practitioner meaninful information about their pain. The tool below was developed at the 10 High St Pain Management Clinic. It has been used by several thousand patients and is now widely used by Australian Pain Practitioners.
